Albany Office
Monday - Thursday 8:30 a. m. until 5:00 p. m.
Friday 8:30 a. m. until 4:00 p. m.

Cordele Office
Monday, Tuesday and Friday 9:00 a.m. until 4:00 p. m.

APPOINTMENTS
Appointments are required at both of our office locations. The doctor that sees patients in a particular location may vary by day. Please specify if you wish to see a specific doctor so that we may ensure you are scheduled appropriately. If you are unable to keep your appointment, please call our office as soon as possible in order to reschedule to a new day and/or time.

We would appreciate at least 24 hours notice.

EMERGENCIES
Our doctors are on-call for our current patients 24 hours a day. If you are not currently a patient of our office, then you would be referred to the ophthalmologist on-call.

BILLING & FINANCIAL
Our practice billing policy is that payment is due the day of your office visit.

Prior to your appointment, please check with your insurance company about specific coverage or limitations that may apply to your office visit. Some companies require that you see a specific doctor or may require a referral from your primary care doctor before you see one of our doctors.

Please bring your insurance card to your appointment. If your health insurance coverage changes, please let us know as soon as possible.

We do not file all insurance plans. Please ask when your appointment is made if we file your particular plan for you. You will be informed at the time of sign in if we do not file with your plan. You would then be expected to pay on that day and then submit to your insurance company for reimbursement. We will provide you with the necessary information to file after payment is received.

We are providers for many insurance plans such as Blue Cross / Blue Sheild, Aetna, 1ST Medical Network, Secure Health, PHCS, Medicare and Medicaid. We will file for your office visit or surgical procedure according to the terms of our contract but you as the patient will be expected to pay any co-pay, deductible or co-insurance that is required on the day of the visit or procedure. Any amount that is not covered by your insurance plan will be billed to you with 90 days to settle the outstanding balance.

PRESCRIPTION REFILLS
All prescriptions and authorization for refills should be requested during normal office hours.

RELEASE OF MEDICAL RECORDS
To protect your confidentiality, medical records are only released with your written consent and your balance paid in full according to our Privacy Policies.
